Can medication reconciliation protect you from drug errors?

Each time your doctor prescribes a new medication, you have the responsibility of learning about the purpose of the prescription you are taking, how to take it and what negative side effects to watch for. Fortunately, with a process called medication reconciliation, you can aid your health care provider in New York to make the right decisions about the prescriptions you are given. In fact, with this method, you can significantly reduce your chances of being the victim of a preventable medication error. 

According to the Mayo Clinic, medication errors are often caused by lapses in communication, confusion about labels or doses, and medical abbreviations that may be similar to each other. Each time you are prescribed a new medication, you should go through a process called medication reconciliation where you and your doctor discuss everything you are currently taking. This assessment will provide your doctor with the information needed to make an informative decision regarding your treatment.

  • Which medications you are allergic to.
  • The name of everything you are currently taking, as well as supplements. You should also disclose dosage amounts.
