Why do wrong-site surgeries occur?

According to information published on the website of the Agency for Healthcare Research and Quality, there are a variety of factors that play a role in wrong-site surgeries, such as poor communication between surgical team members and the failure to review medical records properly. Wrong-site surgeries and other surgical errors can result in serious injuries and loss of life, which highlights the importance of understanding what causes these mistakes and holding incompetent physicians accountable.

On top of the aforementioned factors, there are a number of other reasons why wrong-site surgeries take place. For example, some facilities may be understaffed or fail to provide sufficient training. Also, medical workers do not always use a sufficient system to make sure the procedure is performed on the proper site or use a checklist to ensure that nothing was skipped.

Sometimes, time constraints can result in wrong-site surgeries and may be caused by an overwhelming schedule or an influx of unexpected emergencies. Furthermore, some procedures require equipment that a physician may not have a great deal of experience with. Other factors include multiple surgeons working on the same procedure, improper marking of the operation site, ineffective communication and failure to assess a patient properly.

Although wrong-site surgeries are rare, these errors may be considerably under-reported and more prevalent than many people realize. Additionally, more wrong-site surgeries have been reported in recent years.
